Annual Awards Nomination Forms

The MWCC will be presenting three awards at this year's Annual Meeting on November 3, 2011: Member Company of the Year, Lean Leader of the Year and Volunteer of the Year.  The winners will be chosen based on the degree to which the nominees meet award criteria, as described in their nomination submissions.  Nomination forms can be found below:

The MWCC must receive all nominations by October 7, 2011.  A committee selected from among the MWCC Board of Directors will judge nominations and the winners will receive an engraved award.  The MWCC will confirm all nomination receipts via email.
